Contents of the advance directive
iDecide guides you module by module through your advance directive. Short texts and animated videos explain each topic. You don't have to fill in all the modules at once. iDecide is based on the FMH advance directive and was developed with the University Centre for Palliative Care at Inselspital, Bern University Hospital.
Appoint a healthcare proxy
You decide who will make decisions in line with your wishes if you lose decision-making capacity. A communication guide helps you discuss the important questions with your healthcare proxy.
Set out your attitudes to life
Your values, fears and priorities help your healthcare proxy and the medical team decide in line with your wishes.
Define medical procedures
You set your preferred treatment goal: whether you want resuscitation, intubation and treatment in intensive care, both for acute and for permanent loss of decision-making capacity.
Decide on organ donation
You record whether or not you want to donate your organs.

You fill in the modules in the app or in your browser, at your own pace. You can stop at any time and continue later.

Sign
You print the PDF and sign and date it by hand. Only then is your advance directive valid.

Your advance directive as a QR code
The QR code makes your advance directive quickly available in an emergency. It can be read with any smartphone camera, without an app. It shows your decisions on medical procedures and your healthcare proxy's contact details. The data is contained in the code and is not stored on a server.
