How iDecide supports you

iDecide is an introduction to advance care planning. The app explains what an advance directive contains in plain language and short videos, and guides people step by step through the decisions.

This saves healthcare professionals time: your patients already know the basic concepts of advance directives and possible medical procedures. In the follow-up conversation, you can focus on specific questions and on the advance directive in light of your patient's health situation.

Your patient can share the completed advance directive with you directly from the app, including a scan of the signed version. This makes it easy to file in the medical record.

How best to use iDecide

  1. Recommend iDecide

    Recommend iDecide, for example with the flyer. The app is free, needs no account and also works on a computer.

  2. Preparation at home

    Your patient fills in the modules in their own time and discusses their wishes with their healthcare proxy.

  3. Explore together

    In a conversation with you, open questions are clarified and decisions are adapted to your patient's personal medical situation.

What the view shows

  • Name, date of birth and place of residence, date of creation and signature
  • Decisions on resuscitation, intubation and intensive care, separately for acute and permanent loss of decision-making capacity, and whether the healthcare proxy may deviate from them in unforeseen situations
  • Healthcare proxy with phone number, any additional healthcare proxy and the GP
  • Organ donation and additional remarks

The data is fully contained in the QR code and is not stored on a server.
